HOW LONG IS THE BARIATRIC EVALUATION?

Approximately two, 1 hour appointments. These appointments are available virtually or in-person.
As a therapist and a bariatric patient, I have much to share to help prepare you for your journey.

WHAT CAN I EXPECT?

We will discuss  variety of things including your medical history, mental health history and why you are choosing bariatric surgery.  More importantly, we will discuss bariatric planning & support.  I will provide some education on the importance of mindset work as it related to eating patterns/habits as well as patterns of behaviors to look out for as you move through bariatric life.

DO I NEED TO BRING ANYTHING?

Fees for service is due at the time of the appointment, including any applicable co-pays or high deductibles.

IS THERE ANYTHING I SHOULD DO TO PREPARE FOR MY APPOINTMENT?

  • Complete any paperwork that was sent to you via email from SimplePractice. If you have not received it, please email me. Paperwork that is not completed within 24 hours of the scheduled appointment is subject to cancellation.
